How do I keep my coverage after I leave employment? If you’ve had at least one premium payroll deducted before leaving your job, we’ve got you covered. Your coverage will automatically switch to direct billing, and you’ll receive a bill from AFR Life. To continue your coverage, simply pay us directly. If you decide not to keep the coverage, you can either let it lapse or call us to cancel at 800-425-9303.
How do I set up my policy on automatic bank draft? Download the AFRL Bank Draft Form. Once completed, mail it to  P.O. Box 21061, Oklahoma City, OK 73156, or fax it to 580-225-0951.
How do I file a claim? Give us a call at 800-425-9303 to speak with a Claims Advocate, who will guide you through the process and answer any questions. Complete the AFR Life Claim Form and mail it to P.O. Box 21061, Oklahoma City, OK 73156, or fax it to 580-225-0951.
How do I make changes to my policy? If you have questions about what options are available, please call us at 800-425-9303. You can also review the Policy Change Form before contacting us.
How do I change my beneficiary information? Please complete the AFR Life Worksite Change Form. Once finished, mail it to  P.O. Box 21061, Oklahoma City, OK 73156, or fax it to 580-225-0951.
